Processing Instructions:

  1. Fill out form on computer.
  2. Print form, sign and date it.
  3. Fax completed form to 310-265-6520.

The best and most secure hours for completed forms to be faxed are 9:00 AM to 3:00 PM PST, Monday through Friday. The fax machine is located in a open area and to guarantee security of personal information, forms should be submitted during the 9:00 AM to 3:00 PM time frame.
